Sorry to be out of touch for a bit – thank you both! Incredibly helpful perspective.

So, I think for an addendum (which is what this would be), you would want it shorter and more to the point. I think the second two paragraphs are fine, but I think the first paragraph is too much detail. It's well-written and affecting so I don't think it would hurt, necessarily, but my understanding is that an addendum is generally brief (and frankly kind of dry).
I think this is generally correct. I had some C&F issues that I was worried about (just multiple tickets stemming from partying a lot as a stupid college kid), so I consulted a Spivey-like professional about writing my addendum (overkill, but I was paranoid), and this was the advice that I was given.

So, I'd co-sign all of Nony's advice.

So for instance, I would cut out the descriptive stuff and do something more like this (not claiming this is great writing and you should put it in your own words):

"On [date], just before my second semester of college, my mother committed suicide. As a result, I developed a serious depression that interfered with my ability to successfully complete my classes. After failing to completely the minimum number of credits that semester, [then the rest of the two paragaphs]."

(I'm happy to delete that later if you'd rather not have that much info up on the site.)

I say this because I don't think you need to go into what your mother struggled with, nor do I think you need to expand on details about how it affected you. She committed suicide; you struggled in the aftermath; that's totally understandable and I don't think you need to paint a picture of exactly how to provide info that the adcomms need to know.

I do have one question, though - I'm a little confused about the timeline and if the Fs are after you returned to school after doing therapy? B/c as written above, your addendum makes it sound a little like you were suspended, got your act together, and came back and did fine after. But your grade timeline with the Fs doesn't quite add up to that. So it might be worth changing "markedly improved grades" to something about steady improvement/an upward trajectory?

GPA Addendum?

This is probably the fulcrum of my whole application package, and I can't stop obsessing over it. I'm simultaneously worried this is too long and also that it leaves unanswered questions, but without a stranger's perspective I'm not sure which questions. The second paragraph is meant to be responsive to "C&F" and "interruption of studies" addendum requests, which lots of schools seem to require. I'm trying to cabin this topic off into an addendum, so that the rest of the app can be more positive, and I would also appreciate feedback about that approach. Thanks as always.

Note: this is SFW but discusses mental health issues and suicide.
